The research explores the main causes of failures, such as software development errors and integration issues between components, as well as analyzes their impacts on industrial processes and overall project performance. Based on a literature review and case studies, effective strategies and tools for risk mitigation were identified, such as integrated testing, technical validations, the use of specific standards (IEC 61508, IEC 61511, and ISO 31000), and engineering best practices. The study also emphasizes the importance of detailed planning and continuous monitoring throughout all stages of the systems' life cycle. The results indicate that proactive risk management significantly contributes to cost reduction, increased operational reliability, and the prevention of critical failures.
